Application Process:
The rotation application and supporting documents for clinical rotations should be received by the Physician Education department at least 30 days before the desired rotation date. Once a rotation date is approved and granted, all required compliance training modules and attestation forms should be submitted to the Physician Education department prior to the start of the rotation. Rotations may not start on a holiday or weekend if student has not registered and/or completed orientation with the Physician Education Department.
Documents required of rotating US Medical Students
- Current Kaiser Permanente Rotation Application*
- Letter of Agreement or Good Standing from Medical School Dean/Faculty Member stipulating dates approved as well as malpractice and liability coverage*
- Current health screening w/proof of PPD and MMVR (within 1yr)
- State Issued Photo ID
* This document is not required of students/residents from Kaiser Permanente, Southern California affiliate learning institutions applying for core curriculum or clerkship rotations. Medical Schools with a current Letter of Understanding (LOA), Memo of Understanding (MOU) or contract for clinical education can submit a list of medical students approved for clinical rotation in lieu of each student submitting a rotation application and Letter of Agreement/Good Standing. Students applying for elective rotations apart from the medical school must submit all documents listed above.
**These are minimum document requirements, please inquire with specific location contact for additional location requirement(s).
The medical school’s list of rotating students should be submitted at least four weeks before the rotation start date and include:
- signature of the Clerkship Director/faculty member,
- the requested duration of rotation,
- a statement regarding the students approved standing and the institutions responsibility for malpractice and liability coverage.
Before the Rotation
After medical students are approved for a clinical rotation, each student will be required to complete/submit the following documents before the start of the rotation. It is recommended to have the following documents completed before the orientation day and rotation begins:
- Kaiser Permanente Compliance, HIPAA Training and Course Completion Form
- Current Health Screening w/proof of PPD & MMVR (within 1 yr)
- Confidentially Agreement
- Child Abuse Reporting Form
- Elder Abuse Form
- Alcohol and Drug Policy
- EMTALA Guidelines Form
- Acknowledgement & Attestation Form
- Guidelines for Universal Safety Precautions
- Rotator Information Sheet
- Copy of State Issued ID
Orientation Day
All clinical rotators are required to check-in at Kaiser Permanente Physician/ Staff Education Department on the first and last day of the rotation. The first day of the rotation will include a facility orientation. The Physician Education department coordinator will make a copy of the state issued photo ID, collect any required documents including proof of health screening, and ensure the rotator has completed the Kaiser Permanente Compliance documents and training. Once this process is complete the rotator will be provided a badge with access to the hospital campus and parking facilities.
Conclusion of the Rotation
All clinical rotators are required to check out of the rotation at its conclusion and return any Kaiser Permanente property, clothing, and badges. An exit evaluation form may be requested of each rotator.

- CA State Medical License-Required of American Medical Graduates after 2nd year of residency International Medical Graduates after 3rd year of residency
- DEA Certificate- Required of American Medical Graduates at the start of 3rd year of residency, International Medical Graduates at the start of 4th year of residency
- Current BLS or ACLS Certification
- State Issued Photo ID
- Current Health Screening w/proof of PPD/MMVR (within 1yr)

Affiliate learning institutions with a current Memo of Understanding (MOU) or contract for clinical education can submit a list of residents approved for clinical rotation in lieu of each student submitting a rotation application and Letter of Agreement/Good Standing. Residents applying for elective rotations apart from the learning institution must submit all of the above documents.

Sign-In Procedure
All resident/affiliate rotators are required to submit a rotation schedule. The schedule must be signed by the resident/affiliate rotator and the physician preceptor at the conclusion of the rotation.

All rotators must check out of the rotation at its conclusion and return all Kaiser Permanente property, clothing, and badges.
An exit evaluation form may be requested of each rotator.
Process for International Medical Students

If you are an International Medical Student
Kaiser Permanente is not currently processing rotation requests for international students or residents at our medical center locations.
International students can apply for a rotation through the David Geffen School of Medicine at UCLA. Rotations are only offered at UCLA-affiliated Kaiser Permanente locations (which include Los Angeles and Woodland Hills only), and students must meet UCLA eligibility requirements for international visiting medical students.

Observerships within Southern California, Kaiser Permanente medical facilities and hospitals are generally not granted. Special circumstances can be reviewed on a case by case basis and must be presented and approved by the Regional Director, of Physician Education and the local Medical Director prior to the start of the rotation.
Special Circumstances include:
- Persons who are currently enrolled in a physician training program or medical school and are covered under a contractual agreement with Kaiser Permanente for educational rotations
- Residency trained licensed physicians who are part of a selection process or interview and have completed the required documents for rotating affiliates. Persons in this group are not permitted to provide patient care, prescribe medications, or perform clinical or surgical procedures.
- residency trained, licensed physicians who are involved in a contractual agreement for research

Each patient seen by an observer must consent in writing. The consent forms must be kept in the patients file and in the file of the observer. Observerships are not graded and are not counted toward curriculum credit. Approved observers must complete the facility orientation for rotators with Physician Education and receive a badge from KP Security Services. The rotation application and supporting documents for clinical rotations should be received by the Physician Education department at least 30 days before the desired rotation date. Rotations may not start on a holiday or weekend if student/resident has not registered at the Physician Education department.
Documents required of approved, rotating Observers:
- Current Kaiser Permanente Rotation Application
- Current CV/ERAS application
- Letter of Agreement or Good Standing from Medical School Dean/Residency Program Director confirming observer has completed appropriate level of training
- State Medical License-Required of American Medical Graduates after 2nd year of residency, International Medical Graduates after 3rd year of residency
- DEA Certificate- Required of American Medical Graduates after the 2nd year of residency, International Medical Graduates after 3rd year of residency
- Current BLS or ACLS Certification
- State Issued Photo ID
- Current Health Screening w/proof of PPD/MMVR (within 1yr)
Rotation Orientation
First Day of the Rotation: Each rotator must report to the Physician Education office on the first day of the rotation for medical facility orientation, ID badge, and medical center safety information. This process can take up to 1-2 hours depending on the status file. All Rotators must be informed of the local medical center safety procedures. Rotations may not start on a holiday or weekend.
Rotating Residents and Affiliates Sign-In Procedure
Clinical observers are not required to sign in or submit a rotation schedule to Physician Education.
Conclusion of the Rotation
All rotators must check out of the rotation at its conclusion and return all Kaiser Permanente property, clothing, and badges.
