Application Checklist

• ERAS Application
• Personal statement (one page only, 10-12 point font, single space).
• Curriculum Vitae.
• USMLE Part 1,2, and 3 scores.
• Medical school transcripts.
• Dean's Letter.
• Residency Program Director’s letter of recommendation.
• Three other letters of recommendation, including one from a non-cardiologist.
• If applicant has rotated through a Kaiser Permanente facility, an optional letter from a Kaiser Permanente attending physician is an added plus.
