Application Checklist

This will be indicated on the ERAS application.

• ERAS application
• Personal Statement (one page only, 10-12 point font, single space).
• Curriculum Vitae.
• USMLE Part 1, 2, and 3 scores.
• Medical school transcripts.
• Dean's Letter.
• Residency Program Director's letter of recommendation.
• Two other letters of recommendation.
• If applicant has rotated through a Kaiser Permanente facility, an optional letter from a Kaiser Permanente attending physician is an added plus.

To participate in the fellowship, the physician must have:

• Completed an ACGME-accredited Family or Internal Medicine Residency Program
• California Medical license  
• Current DEA, BLS, ACLS certificate
• USMLE transcripts for Step 1,2,3 Board scores
• 3 Letters of Recommendation (1 must be from Program Director)
