Facilities
The Kaiser Permanente Fontana Medical Center is the primary hospital where
fellows obtain their acute inpatient experience. However, fellows may also
obtain clinical experience from the Community Extended Care Hospital of
Montclair, Inland Valley Nursing Home, Reche Canyon Rehabilitation Center,
Citrus Nursing Home, and/or the Continuity Geriatric Clinic.
The Fontana Medical Center serves a regional population of more
than 500,000 people, including a large geriatric membership, many
of whom are very frail with frequent hospitalizations. The
fellow will interface with many of these patients, from requests
for inpatient geriatric consultations or for pain consultations. Many
of the geriatric syndromes and issues are seen in the context of
the acute hospital, which serves an invaluable role in residents’ geriatric
education. A large proportion of patients referred for pain
consultation are in fact elderly with other geriatric issues other
than pain that are addressed in the process of the evaluation.
Fellows see three to four patients each week
in the Geriatric Assessment clinic. They will also see 4-5 patients each week as outpatients while on the Palliative Medicine rotation.
