How to Apply

To participate in the fellowship, the physician must have:

• Completed an accredited Orthopedic Surgery Residency Program
• California Medical license
• Fluoroscopy license
• Current DEA certificate
• USMLE Step 1,2,3 Board scores

To apply for the fellowship send:

1. Curriculum vitae of your educational, work and research experiences
2. Four letters of recommendation (1 letter required from Residency Program Director)
3. Universal Application
