Documents Required

In addition to the Kaiser Fellowship application, the minimum required documents are:

• Personal Statement
• Curriculum Vitae
• USMLE/COMLEX board scores
• Medical School transcript
• Program Director's Recommendation Letter
• Letter of good standing from current residency program
• Letters of recommendation from three medical school faculty members and/or clinical rotation preceptors
• If applicant has rotated through a Kaiser Permanente facility an optional letter from a KP attending physician is an added plus
