Application Checklist

• ERAS application
• Dean’s letter
• Three letters of recommendation, with at least one letter relevant to the specialty of interest
• If applicant has rotated through a Kaiser Permanente facility, an optional letter from a Kaiser Permanente attending physician is an added plus
• Medical school transcripts
• USMLE Part 1 scores
