Applications are accepted through the Electronic Residency Application Service (ERAS) at http://www.aamc.org/students/eras/start.htm

Documents required:

• ERAS application
• Personal statement
• Dean’s letter
• Three letters of recommendation – one Family Medicine, two others.
• If applicant has rotated through a Kaiser Permanente facility, an optional letter from a Kaiser Permanente attending physician is welcomed
• Medical school transcripts
• USMLE transcripts with Part 1 & 2 scores
