• ERAS application
• Personal Statement
• Curriculum Vitae
• Dean's letter
• Medical school transcripts
• USMLE transcripts/board Part 1 and 2 scores
• Three letters of recommendation, with at least one from physician in specialty of interest
• If applicant has rotated through a Kaiser Permanente facility, an optional letter from a Kaiser Permanente attending physician is an added plus
